Specialty: Family Medicine
Dr Tan Chee Wei is an associate consultant family physician with the Department of Care and Health Integration at Changi General Hospital (CGH). Since graduating from NUS medical school in 2005, he has gained work experience from working under various clusters of primary care in Singapore namely SingHealth Polyclinics, National Healthcare Group Polyclinics as well as the Family Medicine Center and private practices before joining CGH in 2015. He is now involved in care integration and reconciliation of medical care within the hospital and is also actively involved in community healthcare by working closely with the community nurses to improve care transitions from hospital to home.
Dr Tan is interested in medical education, being appointed tutor as well as examiner for the Graduate Diploma of Family Medicine program with the College of Family Physicians Singapore. He is also appointed a member of the adjunct teaching faculty for the Lee Kong Chian School of Medicine.
