Our Medical Records Office (MRO) provides a one-stop service for medical report requests.
In SNEC, medical records are kept strictly confidential. Access to your medical records can only be released with your written consent, unless required for police investigation purposes or by Ministry of Defence. The release of medical information is subject to approval by the Centre.
How do I request for a medical information?
To request for medical information, kindly ensure that:Patient’s written consent is required for the release of medical information. Patient must be aged 21 years and above.
For patients below 21 years old, the consent of the patient’s parent or legal guardian is required. Please attach scanned/photocopy of patient’s birth certificate and parent’s/guardian’s NRIC (front and back view).
If the patient is deceased, a copy of the patient’s death certificate and other documents e.g. marriage certificate, birth certificate, letters of administration as proof of your relationship to the deceased must be submitted together with the application.
If the patient is mentally incapacitated, the written consent of his/her authorised representative or all his/her immediate next of kin must be obtained for the release of the medical report.
The request for medical information can be made:

Please submit email application to mro@snec.com.sg together with the completed Consent for Release of Medical Information Form and scanned copies of your NRIC (front and back view).
If you are below 21 years of age, please also attach scanned copies of your birth certificate and the NRIC (front and back view) of parent/guardian.
For insurance claim, please obtain an Insurance Claim Form from insurance advisor/company and enclose the claim form with the authorisation section fully completed and duly signed.
Please pay by Credit Card and complete the Mail Order Facility Form.
3. By Post
Please download and complete the Consent for Release of Medical Information Form. All sections in the form must be fully completed and duly signed.
Please attach a photocopy of your NRIC (front and back view) / identification documents and enclose a crossed cheque made payable to "Singapore National Eye Centre Pte Ltd". Please indicate patient’s name and NRIC No. at the back of the cheque.
For credit card payment, please complete and attach the Mail Order Facility Form
How much do I need to pay and how soon can I obtain the requested medical information?
As a general guide, the charges and time frame for completion of medical reports are indicated below. The fees below do not include Laboratory charges / X-ray charges / Facility Fees.However, we may experience a delay in processing some requests due to the following reasons:
The application form is incomplete and / or supporting documents required for verification are not produced
Patient has multiple reports requested from several clinical departments
Patient has upcoming / frequent clinic appointments
Patient has been admitted to the ward
Doctor is away
How will I receive the medical report / insurance form?
You can request for the medical report / insurance form to be:
Delivered by ordinary or registered mail
Mailing address to be provided in your application form. Please note the mailing charges for the different types of delivery services.
Self-collection
Please indicate your contact number in the application form. You will be contacted once the report is ready for collection. At the point of collection, your/patient’s original identification card will need to be produced for verification purpose.
Collection by representative
Authorisation Letter with the representative's name and NRIC/Fin/Passport number as well as a copy of the patient's NRIC/Fin/Passport has to be provided if a third party is collecting the report on patient's behalf.
Why do I need to pay for a medical report / completion of insurance form?
The regular consultation and hospitalisation fees paid do not include cost for a medical report as not every patient requires one.
A fee has to be levied for this additional service of providing medical reports to cover the hospital’s administrative costs and the doctor’s professional input. This is standard practice across all restructured hospitals in Singapore.
